Driver's Crash Report

Welcome to the Nebraska Department of Transportation (NDOT) Crash Reporting webpage.

This page is designed to be used on desktop/laptop types of personal computers. Using cell phones or other mobile devices may result in unanticipated issues.

If the crash was not investigated by a law enforcement officer, state statute requires every driver of a motor vehicle involved in a crash resulting in either injury, death, or estimated damages equal to or exceeding $1,500 to the property of any one person (including the driver’s property) to complete and submit a confidential Driver’s Motor Vehicle Crash Report within 10 days following the crash. Drivers involved in motor vehicle crashes where an Investigator’s Crash Report is completed by a law enforcement officer do not need to complete a crash report.

This website will allow you to digitally submit a Crash Report document to the Nebraska Department of Transportation. This webpage is designed to be used on desktop/laptop type of PCs. Using cell phones or other mobile devices may result in unanticipated issues.
